Process for Requesting Medical Certificates
Outpatient

    Once the physician types the report, it will be printed and stamped.

    Pay for the medical certificate and get the hospital seal.
Inpatient

    Notify & request to the nurse 1-2 days before discharge.

    On the day of discharge, pay for the medical certificate and get the hospital seal.
In accordance with Article 17 of the Medical Law, a medical certificate cannot be delegated if the patient is conscious, and can only be issued directly to the patient.

Certificates that Require Department Check-In at the Payment Desk
Diagnosis Report (KOR/ENG), Medical Report (KOR/ENG), Proof of Vaccination (KOR/ENG), English Birth Certificate, Referral Report, Image CD Copy, Pathology Slides

Certificates that can be Issued from the Payment Desk
Certificate of Outpatient Consultation, Certificate of Admission, Copy of Korean Birth Certificate, Certificate of Inpatient Room Stay, Itemized Bill (Outpatient/Inpatient), Proof of Outpatient Payment

About Requesting Medical Certificates
If requesting medical certificates on behalf of a patient, the following documents are required:
Consent form, Power of Attorney (original) signed by the patient. (Stamp & fingerprints will not be accepted)
Certificate of Proof of Family. Must be issued within 6 months.
In accordance with Article 17 and Enforcement Rule Article 13 (3) of the Medical Law, without all the required documents, medical certificates cannot be issued.
Type of identification:
  • Alien registration card, passport, driver's license, or government issued photo ID.
  • Under the age of 17 : passport, student card, certificate of proof of family.

    • Official medical report stating that patient is unconscious, or has a serious illness (injury) preventing patient to sign consent form
    Missing Person
    • Applicant’s photo ID (immediate family member)
    • Proof of Family (i.e. birth certificate)
    • Official document filed for missing persons
    Patient with difficulty in communication due mental/physical disability
    • Applicant’s photo ID (immediate family member)
    • Proof of Family (i.e. birth certificate)
    • Physician’s written opinion of patient’s inability to communicate.
    Comments
    • For siblings requesting, the following documents are needed:
      • Patient’s proof of family (i.e. birth certificate)
      • Parent’s proof of family
      • Consent form
    • For power of attorney requesting, the following documents are needed:
      • Power of Attorney’s ID
      • Power of Attorney document

Medical Certificates issued via internet
Certificate of Outpatient Consultation, Certificate of Inpatient Admission, Outpatient Receipt, Inpatient Receipt, Copy of Korean Birth Certificate, English Birth Certificate (original), Outpatient Itemized Bill, Inpatient Itemized Bill, Copy of Medical Report (Korean/English), Copy of Medical Opinion

Precautions
The fee for issuing a medical report and/or written opinion is not covered by health insurance and consultation fee is calculated together with the medical report fee.
The diagnosis and diagnosis code are not included in the outpatient medical certificates when issued via internet. If you need a document with the diagnosis and diagnosis code, please request it directly from your physician.
