What are medical records?
It is a document that records the patient’s personal information and disease (current medical history, history of past disease, history of surgical treatment, etc.) and every treatment the hospital has conducted.

Therefore, medical records are highly sensitive information that should not be known to others. In this regard, the current medical law stipulates the protection of personal information of medical personnel, and in principle prohibits the viewing and issuance of copies of medical records to others other than patients.

However, as an exception, a copy of medical records can be issued to the patient's spouse, lineal ascendant, descendant, and his or her designated representative only if they provide the required documents specified in the medical law and obtaining prior consent from the patient. In addition, as the patient’s medical information is sensitive, any personal information cannot be given over the phone.

Required documents for requesting medical records
  • The consent form and power of attorney must be the original form, and the signature for each form must be handwritten. (Stamps or fingerprints are not accepted)
  • Required documents: Proof of Family (i.e. birth certificate) / Documents must be issued within the last 6 months to be accepted.
  • In accordance with Article 17 and Enforcement Rule Article 13 (3) of the Medical Law, without all the required documents, medical certificates cannot be issued.
Identification
  • Alien registration card, passport, driver’s license, or government issued photo ID
  • For age 17 and under : passport, student card
Power of Attorney
The patient's consent form or power of attorney should be written in specific details and must include the patient's signature
(unless the patient is 14 years or under).

  • Weekdays 08:30 ~ 17:30 (Lunch hour 12:30 ~ 13:30)
  • Saturdays, holidays (morning consultations) 08:30 ~ 12:30
  • Must request no later than before 30 minutes to closing.
